Drug Description Fentanyl Transdermal Patch
Fentanyl Transdermal System (Patch) is an opioid used as a pain medication and together with other medications for anesthesia. Medically, Fentanyl is used by injection, as a patch on the skin, as a nasal spray, or in the mouth. Fentanyl patches are on the WHO list of essential medicines, the most effective and safe medicines needed in a health system. They are used in chronic pain management. These patches work by slowly releasing fentanyl through the skin into the bloodstream over 48 to 72 hours, allowing for long-lasting pain management. Dosage is based on the size of the patch, since, in general, the transdermal absorption rate is constant at a constant skin temperature.
Fentanyl transdermal system is indicated for the management of pain in opioid-tolerant patients, severe enough to require daily, around-the-clock, long-term opioid treatment and for which alternative treatment options are inadequate.
Do not use the patch form of Fentanyl to relieve pain that is mild or that will go away in a few days.
Dosage and Administration
Due to the risk of respiratory depression, fentanyl transdermal system is only indicated for use in patients who are already opioid-tolerant. Discontinue or taper all other extended release opioids when beginning treatment with this medication.
Patients considered opioid-tolerant are those who are taking at least 60 mg of morphine daily, or at least 30 mg of oral oxycodone daily, or at least 8 mg of oral hydromorphone daily or an equianalgesic dose of another opioid for a week or longer.
Initiate the dosing regimen for each patient individually, taking into account the patient’s prior analgesic treatment experience and risk factors for addiction, abuse, and misuse.
Side Effects of Use
Common side effects include addiction, abuse, respiratory depression, hypotensive effects and serotonin syndrome.
